AN ORDINANCE AMENDING Section 25-9-1, of the Revised Ordinances
of Salt Lake City, Utah, 1965, relating to Salt Lake City employee
sick leave benefits and procedures.
Be it ordained by the Board of Commissioners of Salt Lake City,
Utah:
SECTION 1. That Section 25-9-1 of the Revised Ordinances of
Salt Lake City, Utah, 1965, relating to Salt Lake City employee
sick leave benefits and procedures, be, and the same hereby is
amended as follows:
Sec. 25-9-1. Salt Lake City employee sick leave benefits
and procedures.
(1) Sick leave shall be provided for all permanent
permanent full or part-time employees only, as insurance
against loss of income when an employee is unable to perform
assigned duties because of illness or injury.
(2) Each officer and full time employee of Salt Lake City
shall be entitled to fifteen days sick leave each calendar
year, except members of the combat division of the fire
department who shall be entitled to seven and one-half on-
duty shifts of sick leave each calendar year.
(3) No officer or employee of Salt Lake City shall be
entitled to the foregoing sick leave, until such person has
completed six months of continuous employment with Salt
Lake City.
(4) Officers and employees absent from work on account
of illness or injury shall report intended absence to their
department head or supervisor as soon as practable but in
no event later than fifteen minutes after commencement of
the employee's duty shift.
(5) Sick leave in excess of two days (or one on-duty
shift in the case of fire department combat division
personnel) for any one illness shall be documented by a
written certification from a health department nurse or
a licensed practicing and attending physician, that during
the period of leave, the absent officer or employee was
prevented by illness from discharging the duties required
by his office or position of employment. Such certification
shall also be required of any employee claiming sick leave
benefits for an absence not reported in compliance with
subsection (4) hereof or at such other times at which
verification is requested by a supervisor or department
head.

(6) A health department nurse shall be available,
at the request of employees or their supervisors, to
provide examination or certification under the provisions
of this section.
(7) The following disciplinary action shall be deemed
manditory for abuse or neglect of sick leave privileges
or procedures, except where the officer or employee
demonstrates justifiable excuse to the department head:
(a) For failure to timely report absence from
work as required in subsection (4) of this section - a
minimum of four hours suspension without pay.
(b) For all other abuses of sick leave privileges a
a minimum automatic five day suspension without pay,
together with such further disciplinary measures, including
suspension or dismissal, as may be deemed appropriate by Z
the department head.
SECTION 2. This ordinance shall take effect 30 days after Z
its first publication. in
