AN ORDINANCE AMENDING Chapter 12 of Title 25 of the Revised Ordinances
of Salt Lake City, Utah, 1955, relating to sickness, disability and death
benefits for officers and employees of Salt Lake City, Utah.
Be it ordained by the Board of Commissioners of Salt Lake City, Utah:
SECTION 1. That Chapter 12 of Title 25 of the Revised Ordinances of
Salt Lake City, Utah, 1955, relating to sickness, disability and death bene-
fits for officers and employees of Salt Lake City, be, and the same hereby is,
amended to read as follows:
"CHAPTER 12.
SICKNESS, DISABILITY AND DEATH BENEFITS.
Sec. 25-12-1. Sickness, disability and death benefits for
officers and employees. (a) There is hereby established a
system for the payment to all regularly employed officers and
employees of Salt Lake City, including heads of departments, of
sickness, disability and death benefits to be financed, admin-
istered, and payable upon such terms and conditions as may be
from time to time determined and approved by the Board of Com-
missioners.
(b) As of July 1, 1963, all regularly employed officers and
employees of Salt Lake City, including heads of departments, but
excluding temporary and seasonal employees shall be eligible to
participate in a group insurance plan in conformity with and under
the terms of a contract for group insurance covering the said of-
ficers and employees of Salt Lake City entered into by and be-
tween Salt Lake City and the Lincoln National Life Insurance
Company, a corporation of the State of Indiana, authorized to do
business in the State of Utah, effective as of July 1, 1963, and
as may thereafter be modified by agreement between Salt Lake City
and the Lincoln National Insurance Company, for the payment of
benefits as contained in said contract and any further modifica-
tions thereof, and covering the following basic benefits:
1. Payment of a specified sum to the estate or designated
beneficiary upon death.
2. Payment of a specified sum in case of accidental death
or loss of specified body member or members, where such acci-
dental death or loss of member or members is not compensable
under the workmen's compensation statutes of the State of Utah.

3. Payment of a specified sum per day hospital expense.
4. Payment of specified amounts for surgical expenses.
5. Medical care, diagnostic and laboratory examination and
poliomyelitis expenses as designated in said contract.
6. Additional benefits for elected officials and department
heads and key personnel and dependents of all employees as pro-
vided in said contract.
(c) The cost of maintaining said group insurance shall be
borne as follows:
For the basic benefits referred to in subparagraphs 1, 2, 3,
4, and 5 of paragraph (b) the officers and employees eligible and
electing to be covered thereby shall pay such part of the overall
monthly payment as may be provided in said contract between Salt
Lake City and said insurance company. The city shall pay monthly
the balance of the premium charge. For additional benefits re-
ferred to in subparagraph 6 of paragraph (b) the officers and
employees shall pay the entire cost thereof.
The officers' and employees' payments shall be withheld from
the wages or salaries, Salt Lake City having the administrative
responsibility to make payment to said insurance company; provided
that any officer or employee on leave of absence without pay shall
pay to the city the full amount of the premium due during the
leave of absence for the period he is insurable under the contract,
but upon his return shall automatically be entitled to the insur-
ance on the cost basis above specified.
(d) All officers and employees presently employed and eligible
therefor and having elected, are covered by said insurance plan.
All future officers and employees electing to be covered by said
insurance shall execute an application therefor at the time of
permanent employment. Any presently employed officer or employee
who has not elected to be covered by said insurance plan shall
execute forthwith a waiver upon forms to be furnished, and all
future officers and employees not to be covered by said insurance
plan shall execute a waiver thereof at the time of appointment,
provided that temporary or seasonal employees need not execute
such waiver.
Sec. 25-12-2. Retired officers and employees. Any officer
or employee who has been employed by Salt Lake City for a total
period of fifteen (15) years, or more, and who, while employed,
has elected to be covered by said insurance plan, upon retire-
ment from employment with Salt Lake City may participate in said
insurance plan then in effect upon the terms and conditions set
forth in said plan and contract. Officers and employees who have
been employed by Salt Lake City less than a total of fifteen (15)
years shall not be eligible, upon retirement, to participate in
said insurance plan."
Passed by the Board of Commissioners of Salt Lake City, Utah, this
25th day of July, 1963.
