WHEREAS, under the provisions of Title 43a of the Revised Ordinances of Salt LaKe City, Utah, 1955, Bill No. 82 of 1964, adopted December 2, 1964, relating to ambulances, the Board of City Commissioners is empowered to prescribe rules and regulations governing the operation and maintenance of ambulances to provide safe transportation and specify such equipment and regulatory devices as the Board of Commissioners may deem necessary; and WHEREAS, the Salt Lake City Board of Health and the Police Department have prepared and recommended the rules and regulations following and the Board of Commissioners, after having duly considered the same, finds that said rules and regulations are fair and reasonable and are well calculated to provide safe transportation and satisfactory service to the residents of Salt Lake City: NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COMMISSIONERS OF SALT LAKE CITY that the following rules and regulations be, and the same hereby are, adopted and promulgated as the rules and regulations of Salt Lake City governing the operation and maintenance of ambulances in Salt Lake City under said Title 43a, Revised Ordinances of Salt Lake City, Utah, 1955: A. All private ambulance service authorized for pickup in the City of Salt Lake shall be provided on a 24-hour per day, seven-day per week basis with permitted equipment. All permitted equipment shall be stationed at operating locations approved by the Board of Health. ( ) 2. Equipment under suspension is not eligible for substitution. ( ) 3. All ambulance equipment shall be modern and capable of providing a satisfactory emergency ambulance service. ( ) 4. The ambulance body shall have rear loading facilities with the patient's compartment separated from the driver's compartment. The patient's compartment shall contain two or more exits. ( ) 5. Every ambulance shall have adequate heat in patient's compartment. ( ) 6. Every ambulance shall be equipped with emergency red lights mounted in front and siren as authorized and required for emergency vehicles by the Vehicle Code of the State of Utah. ( ) 7. No ambulance or equipment that have been substantially damaged shall be again placed in service until it has been reinspected. ( ) 8. Ambulance upholstery must be kept in good condition. ( ) 9. Storage facilities for linens must be enclosed, or otherwise protected. ( ) 10. Every ambulance must be equipped with a fire extin- guisher of at least 2-1/2 pounds capacity charged with either dry powder or carbon dioxide, as approved by Fire Department. ( ) 11. Every ambulance shall carry at least the following equipment: a. One guerney or 4-wheel cot with adjustable head position ( ) b. Six complete sets of linen ( ) c. One pillow and one blanket for each cot and/or stretcher ( ) d. Two hand towels ( ) e. A sufficient supply of oxygen, together with oxygen mask and regulators for the administration of oxygen for not less than two hours ( ) -3- f. Two soft restraints ( ) g. Approved resuscitation equipment ( ) 1. Ambu-Rescue Breathing Equipment (Hand-type, manual operated) ( ) 2. E.N.J. Resusciation and Emergency Breathing Equipment (Mech. and Auto.operated) ( ) 3. National Cylinder Gas (Mech. and Auto. operated) ( ) 4. Mine Safety Appliance (MSA) Equipment ( ) h. Airway and "S" tubes. Adult Children ( ) 12. Every ambulance shall be adequately equipped with a medicine cabinet containing first aid equipment. Additional supplies shall include approved leg and arm splints, large emesis basin ( ) 13. Every ambulance must be maintained in a clean, sanitary condition and in good repair ( ) 14. Bed linens must be immediately changed after discharge of a patient ( ) 15. No ambulance shall be used as a hearse 16. Every ambulance shall be designed and equipped to transport at least two stretcher patients at one time ( ) 17. Only one order shall be filled on each private ambulance trip ( ) 18. Every call for ambulance service shall be answered promptly. Patients shall be loaded and transported without being subjected to unreasonable delays ( ) 19. In emergency cases, an operator shall not delay in dis- patching equipment, in giving first aid, or in providing transportation for patients ( ) B. RULES AND REGULATIONS APPLICABLE TO PRIVATE AMBULANCE DRIVERS AND ATTENDANTS 1. An attendant, in addition to the driver, must accompany every ambulance on each call. ( ) 2. Every ambulance driver and attendant shall have completed a course in First Aid at least equivalent to the American Red Cross First Aid Standard Course, and such other train- ing as may be required by the Board of Health. ( ) Names of Ambulance Attendants Certification -4- 3. Ambulance drivers shall limit the use of siren to response to emergency calls and transportation of emergency cases. ( ) 4. Ambulance drivers and attendants shall not smoke while operating ambulance. ( ) 5. Ambulance drivers and attendants, while on duty, shall not use, sell, or be in possession of alcoholic beverages. ( ) 6. Ambulance drivers and attendants shall not use, sell or be in possession of narcotics. ( ) 7. Ambulance drivers and attendants shall wear clean uniforms when handling and transporting patients. ( ) 8. Ambulance management and assigned personnel are jointly responsible for seeing that the above rules and regula- tions are complied with. Lack of compliance will be considered good and sufficient grounds for revocation of permits. ( ) 9. Driver and attendant(s) must present a certificate of health from a licensed medical doctor to show evidence of physical and mental well-being. ( ) 10. The applicant has complied with the City Ordinance and provisions of regulations relating to health, sanitation, and safety so as to properly safeguard the lives and health of the employees engaged therein.
