RESOLUTION
Result
WHEREAS, medical claims and expenses of the Salt Lake City Corporation
Employee Benefit Plan are paid both from contributions from city employees
and funds provided by Salt Lake City Corporation; and
WHEREAS, current financial reports on said plan project a deficit in
said benefit plan; and
WHEREAS, it is therefore necessary to increase the contributions of
city employees in order to meet projected claims and expenses; and
WHEREAS, it is desirable to increase life insurance benefits for city
employees.
NOW, THEREFORE, BE IT RESOLVED by the Board of Salt Lake City Commis-
sioners that commencing September 1, 1977:
(1) The monthly premium contributions of employees covered under the
applicable rules of eligibility of the Salt Lake City Corporation Employee
Benefit Plan shall be as follows:
$ 7.45 Single Employee
10.26 Employee plus one dependent
24.56 Employee plus two or more dependents
(2) The employee contribution rates specified hereunder shall be
reviewed on or about March 1, 1978. If they are determined to be in any
respect unreasonable or excessive, subsequent contribution rates shall be
reduced to the extent necessary to offset the excess contribution.
(3) Employee life insurance benefits shall be increased hereby as
follows:
Elected officials and judges no change
Department heads from $4,000.00 to $5,000.00
Other covered employees from $3,000.00 to $5,000.00
Passed by the Board of Commissioners of Salt Lake City, Utah, this
30th day of August, 1977.
