A. Decisions, Feedback & Information needed by staff
1. Randy Kruger,a retired Air Force Major,works at the Utah Test and Training Range.
He has offered to coordinate a visit to the Range and possible tour of Hill AFB for the
City Council and Mayor's office.Are there any Council Members who would like to
attend?
2. Utopia:The Administration would like to meet with Council Members to provide an
update on the Utopia program. It has evolved since the last update. Also,Utopia
would like to invite Council Members to view a demonstration in Provo during the week
of October 20. Provo has a system currently in operation that is somewhat similar to
what is proposed by Utopia. Council staff has viewed the presentation in Provo and
found that it demonstrates the capabilities and potential of Utopia. Would Council
Members be willing to schedule a site visit to Provo to view the system?
3. Increased residential traffic fines in neighborhoods:When the City Council recently
lifted the moratorium on the Traffic Calming Program,they adopted eleven legislative
intents. One was: "It is the intent of the City Council to enact amendments to provide
for an enhanced penalty for speeding on residential streets that are posted at 25 mph."
Do Council Members have specific ideas regarding enhanced level of fines for
this legislative intent?Do you want Council staff to work with the Attorney's
Office to determine the fine structure?Would you like to have a recommendation
from the Administration?
4.Giant in Our City dinner.The Chamber will be hosting the annual"Giant in Our City"
dinner on Wednesday,October 29 at 6:00 p.m.at the Grand America Hotel.Are
Council Members interested in attending?Depending on the number of Council
Members attending,council staff will register for a table or make arrangements to share
a table with CED.
5. Northwest Quadrant Master Plan:Council Members recently received a copy of the
attached letter and proposed timeline/cost estimate from Mr.Jeff Jonas, Planning
Commission Chair requesting that the City Council consider funding the development
of a master plan for the Northwest Quadrant. Mr.Jonas notes:
A. This area was annexed into the City in the mid-1980s,but the City has never
adopted a long-range plan to govern it.
B. A master plan for the area was drafted in the early 1990s,but was never
adopted by the City Council.
C. The lack of a master plan and planning policies for this area is becoming
increasingly problematic.

Would the Council like to include funding for this master plan as an initiative in
Budget Amendment#2(tentatively scheduled for discussion in November)or
would Council Members prefer to discuss this request further during a work
session briefing?
6.Request for Proposals(RFP)for Audit of the Fire Department:The Council
Subcommittee recommended contracting with an audit or consulting firm for a
management and performance audit of the Fire Department. This would continue the
Council's long-standing practice of contracting for major management and operational
audits. Council staff has prepared the attached RFP. The proposed scope of work is
itemized on Exhibit"A"beginning on page 15,part II. Does the Council wish for staff
to issue the RFP? Does the Council have changes or additions to the proposed
scope statements? No. Councilmember Saxton said she was ok with the scope
but wanted to know if the Council normally asked the Chief if there were any
departments or areas he felt would be advantages for an audit. Ms.Gust-Jenson
said yes. Councilmember Saxton asked if that had been given to them. Ms.Gust-
Jenson said we she had been in contact with the Chief several times as the
proposal evolved. She said department input would be received before
commitments were made.
7.Request for Proposals(RFP)for auditors to conduct limited scope audits:For the next
round of small audits,the Council Subcommittee discussed the approach of selecting
two or three audit firms so that more options are available for matching expertise with
each audit assignment. Attached is a draft Request for Proposal(RFP)to solicit
proposals from interested auditors. Does the Council wish for staff to issue the
RFP? Yes
8.Clarification of Council Staffs Role in coordinating or overseeing audit of
Redevelopment Agency:The City Council,acting as the Board of Directors of the
Redevelopment Agency,approved a management audit of the Redevelopment
Agency. Council staffs understanding is that the Board of Directors asked that Council
Staff coordinate the audit in order to help ensure greater distance and thus objectivity
and independence. Council staff believes that the draft RFP can be strengthened to
clarify that the auditors are to coordinate the audit through the Council Office. In
addition,Council staff has several other suggestions to the wording of the RFP. See
attached RFP with suggestions in revision format.
a. Clarify that"Agency"refers to the Board of Directors and not to the RDA
staff relating to authority to revise the contract,approval of draft reports,etc.
b. Clarify that the contract is to be signed by the City Council Chair rather than
the Mayor since the audit is to be coordinated by the Council Office. (As an
alternative,the RDA staff has indicated that RDA legal counsel could be
asked to determine a way for the RDA Chair to sign; the bylaws currently
stipulate that the Mayor is the official signator for RDA documents.)
c. Clarify that Council staff(audit coordinator)will be a member of the
Committee to assist the Consultant.
d. Require that the proposal content also include any prior auditing or
consulting experience with RDA agencies,the hourly rates for each

individual to be assigned,and the estimated number of hours included in
the not-to-exceed fees.
e. Modify the document to conform to the City's purchasing process for
auditors rather than the process used by RDA to negotiate fees for services
and consulting.

will not be charged for services of the Purchasing Division or City Attorney's Office since

1. Attached is information from Salt Lake Valley Solid Waste Management Council. It
includes 2002 Annual Report,2003 Second Quarter Update,and 2003 Second
Quarterly Statistical Summary for January—June.
2.Water Revenue Forecast:Attached is a memo from LeRoy Hooton regarding water
revenue forecast. Because conservation has been greater than anticipated,revenue
could be down as much as$3.3 million by the end of the fiscal year. The Department
implemented several cost reduction measures:
1. Implemented a soft hiring freeze
2. Avoided the purchase of spot market water
3. Reduced operation and maintenance costs
4. Delayed capital equipment expenditures
5. Delayed upgrade of City Creek Treatment Plant
The good news is that water storage has not been depleted,allowing for carry-over
water for next year.
A briefing will be scheduled in November(per Council Chair)when additional forecast
information is available.

Councilmember Turner was in favor of the master plan. Councilmember Saxton said

INFORMATION& REQUIREMENTS
I. OBJECTIVE. The Salt Lake City Council is soliciting competitive sealed proposals from
qualified firms to conduct a Management and Performance Audit of the Salt Lake City Fire
Department. This Request for Proposal describes the Fire Department, the purpose of the
review, the scope of work, proposal and contract requirements and related information. The
City will consider proposals from national, regional, and local firms.
IL BACKGROUND. The City Council's objective of the Management and Performance Audit is
to address the issue of how effectively the City Fire Department provides services and how well
desired results and benefits are being achieved in the Department's programs. To answer these
questions, the City Council is seeking the services of a qualified Consultant to review the Fire
Department's operations. The purpose of the audit is to assess performance, to identify
opportunities for improvement, and to develop specific recommendations for action. A primary
focus of the audit is to examine Fire Department services against best demonstrated practices
and national standards. This comparative approach is intended to identify opportunities to
increase service levels or operational efficiencies. (See Exhibit A, Scope of Work, for specific
services and requirements that are to be included in the review.)
Salt Lake City Corporation was incorporated January 6, 1851. Salt Lake City is Utah's capital
city, the county seat of Salt Lake County, and the largest city in the four county Wasatch Front
metropolitan area. The City functions as the commercial and industrial center not only for Utah
but for a large portion of the Intermountain West. Salt Lake City is the central city to 1.3
million inhabitants residing within one (1) hour's drive from downtown. The City's population
increases daily from approximately 182,000 to nearly 300,000 as workers commute to jobs
located within the City's jurisdictional limits. The City operates under the Mayor-Council form
of government, with a full-time Mayor elected at-large and seven part-time Council Members
representing specific geographic districts.
The City ranges from residential foothills to a heavily populated high-rise district. It also
includes two large industrial and manufacturing areas. Salt Lake City is protected by fourteen
(14) strategically located fire stations, which are divided into Battalion One (East side) and
Battalion Two (West side). These Battalions provide seven(7)EMT Engines, six(6) Rescue
Engines, three (3) Ladder Trucks, one(1) Hazardous Materials Unit, four(4)Airport Fire
Rescue Units and many other support vehicles. Salt Lake City Fire Department's total manning
is 393 personnel. The Department is divided into two major divisions, the Support Services
Division and the Operations Division. More information can be found on the Fire Department's

III. GENERAL INFORMATION
Offerors should review the required insurance coverage that will be part of the resulting
contract(s). Insurance information is provided under Paragraph 5 of the Sample
Agreement. Proposed pricing must include associated insurance costs. - - -
Interest in alternate fuel use and other environmental matters. The City has an interest in
measures used by service and product providers to insure minimal adverse impact on the
environment. Please list measures such as alternative fuel vehicles, recycling measures and
energy reduction measures used by your firm.

Instructions: When preparing proposals, reply to each of the following proposal content &
evaluation criteria in the order listed. Please restate each numbered point listed below followed
by your response in full, narrative sentences and provide any requested materials:
I. QUALIFICATIONS
A A statement of the firm's experience qualifications and general background to perform
the requested work including the names, titles and telephone numbers of previous clients
who can speak to the firm's ability to perform the work.
B. Identify the audit partners, audit managers, field supervisors and other staff who will
work on the audit. The planned use of specialists should also be specified. Recap the
experience in fire department audits for each senior and higher level individuals that
would be assigned to the audit.
C. Describe your level of knowledge regarding fire department operations and their best
practices and international standards if applicable and recent fire department audits with
similar type and scope as described here within.
D. The location of the office(s) from which the work is to be done.
H. PROPOSED APPROACH TO PROJECT
A. The audit work plan should demonstrate the offeror's understanding of the audit
requirements and the audit tests and procedures to be applied in completing the audit
plan.
B. Provide a tentative time schedule for completing the audit and detail the expected
number and type of audit personnel to be assigned to the project and the number of audit
hours by staff level.
C. In addition to the specific service proposals specified above, identify and/or recommend
any additional services, products, or special knowledge or skills provided by your firm
that may be related to or helpful to a comprehensive Management&Performance Audit
and identify any costs for such services, products, knowledge, or skills. If there is a
better way for the City to package or define the services that it has requested herein in
order to obtain better pricing and/or service by making it easier for the selected
Consultant, please explain what modifications would be necessary to achieve such better
pricing and/or service.

specified including, but not limited to, materials, supervision, labor, transportation, delivery,

D. Electronic payment. The City is encouraging offerors to accept electronic payments using
the process described in Paragraph VII of Attachment 2, General Proposal Instructions&
Information. Please provide information relating to: (1) offeror's ability to accept electronic
payments and remittance advices; (2) offeror's policy, if any, regarding electronic payments
and (3) offeror's discount, if any, for electronic payments.
Selection Process
Following evaluation by the selection committee, two or three finalists may be recommended to the City
Council. At the option of the City Council, the Council may review the proposals of finalists and make
a selection based upon the above evaluation criteria, or the Council may conduct oral interviews with
each finalist. If the Council conducts oral interviews, the Council may evaluate firms based on
professional qualifications, previous experience, proposed work plan, price, and ability to present
information and communicate effectively with the Council. The Salt Lake City Council reserves the
right to reject any and all proposals submitted and to request additional information from all offerors.
Any contract awarded will be made to the firm who, based on evaluation of all responses, applying all
criteria and oral interviews, if necessary, is determined to be the best to perform the audit.

AWARD BY WRITTEN AGREEMENT
The offeror selected to provide the services/products shall be required to enter into a written
agreement in substantially the form as shown in the attached SAMPLE AGREEMENT
(ATTACHMENT 3) which shall be the basic form used to develop the final agreement.
• Signature on the Proposal Cover Sheet acknowledges that the offeror is willing to enter into
the agreement if awarded the contract. Offerors are advised to read thoroughly the Sample
Agreement as the selected offeror will be required to comply with its requirements.
• If offeror has any exceptions to the Sample Agreement, the procedures stated under
Paragraph IV, EXCEPTIONS, of this section must be followed.
• Offerors should pay particular attention to insurance coverage requirements specified
in Paragraph 5 of the Sample Agreement. The selected Consultant will be required to
provide insurance certificates at the time of notification of conditional award.
H. PREPARATION OF PROPOSALS
A. Failure to Read. Failure to read the Request for Proposal and these instructions will be
at the offeror's own risk.
B. Cost Of Developing Proposals. All costs related to the preparation of the proposals and
any related activities are the sole responsibility of the offeror. The City assumes no
liability for any costs incurred by offerors throughout the entire selection process.
III. PROPOSAL INFORMATION
A. Discussions With Offerors. The City may conduct discussions with offerors who submit
proposals determined to be reasonably susceptible of being selected for award.
However, proposals may be accepted without such discussions, at the City's option.
Offeror's costs relating to interviews will be at the Offeror's expense and will not be
reimbursed by the City.

B. Equal Opportunity. The City will make every effort to ensure that all offerors are treated
fairly and equally throughout the entire advertisement, review and selection process.
The procedures established herein are designed to give all parties reasonable access to
the same basic information.
C. Proposal Ownership. All proposals, including attachments, supplementary materials,
addenda, etc., shall become the property of the City and will not be returned to the
offeror.
D. Rejection Of Proposals.
• The City reserves the right to reject any or all proposals received. Furthermore, the
City shall have the right to waive any informality or technicality in proposals received
when in the best interest of the City.
• No proposal shall be accepted from, or contract awarded to, any person, firm or
corporation that is in arrears to the City, upon debt or contract or that is a defaulter,
as surety or otherwise,upon any obligation to the City, or that may be deemed
irresponsible or unreliable by the Purchasing, Contracts & Property Management
Division. Offerors may be required to submit satisfactory evidence that they have the
necessary financial resources to perform and complete the work outlined in this RFP.
E. Failure To Submit A Proposal. Failure to submit a proposal (or to advise the City
Purchasing, Contracts&Property Management Division that future Requests for
Proposal are desired) may result in the removal of your firm from the prospective
offerors list.
IV. EXCEPTIONS TO PROPOSAL& SAMPLE AGREEMENT
If offeror takes exception to any term or condition set forth in this proposal and/or the Sample
Agreement and any of its Exhibits and Attachments, said exceptions must be clearly identified in
the response to this RFP. Exceptions or deviations to any of the terms and conditions must
not be added to the proposal pages but must be submitted in a separate document
accompanying offeror's proposal identified as "Exceptions." Such exceptions shall be
considered in the evaluation and the award processes. The City shall be the sole determiner of
the acceptability of any exception.
Should the City omit anything from this Request for Proposal which is necessary for a clear
understanding of the work, or should it appear that instructions are in conflict, then the offeror
shall secure written instructions from the Purchasing, Contracts&Property Management
Division at least forty-eight (48)hours prior to the time and date of the proposal opening shown
above.
V. CONFIDENTIALITY
All responses, inquiries, and correspondence relating to this RFP and all reports, charts, displays,
schedules, exhibits, and other documentation produced by the offeror that is submitted to the

City, as part of the proposal or otherwise, shall become the property of the City when received
by the City and may be considered public information under applicable law. The City is subject
to the disclosure requirements of the Government Records Access and Management Act, Title
63, Chapter 2, Utah Code Annotated. The City generally considers proposals and all
accompanying material to be public and subject to disclosure. Any material considered by the
offeror to be proprietary must be accompanied by a written claim of confidentiality and a
concise written statement of reasons supporting the claim. Blanket claims that the entire
RFP is confidential will be denied. The City cannot guarantee that any information will be
held confidential. Under Section 63-2-304 of the Government Records Access and Management
Act, if the offeror makes a claim of confidentiality, the City, upon receipt of a request for
disclosure, will determine whether the material should be classified as public or protected, and
will notify the offeror of such determination. The offeror is entitled under the Government
Records Access and Management Act to appeal an adverse determination. The City is not
obligated to notify the offeror of a request, and will not consider a claim of confidentiality,
unless the offeror's claim of confidentiality is made in a timely basis and in accordance
with the Government Records Access and Management Act.
VI. REPRESENTATION REGARDING ETHICAL STANDARDS FOR CITY OFFICERS

The offeror represents that it has not: (1)provided an illegal gift or payoff to a City officer or

VII. GENERAL INFORMATION
A. The City has developed a process by which vendors can receive payments and remittance
advices electronically. The electronic process will (1) allow vendors to receive collected
funds the day following the City's weekly "check write," and (2)will provide an
electronic remittance advice on the day of the "check write." The remittance advice will
be sent via e-mail through the internet to as many as three separate e-mail addresses.
The remittance advice will contain exactly the same information that would have been
included on a check stub. Also available is a vendor web page that allows a vendor, with
proper user name and password, to see its history of payments from the City. The City
encourages offerors to accept electronic payments using this process.
B. Interest in alternate fuel use and other environmental matters. The City has an interest in
measures used by service and product providers to insure minimal adverse impact on the
environment. The City encourages measures such as use of alternative fuel vehicles,
recycling and energy reduction.

MANAGEMENT & PERFORMANCE AUDIT
OF FIRE DEPARTMENT
FOR THE SALT LAKE CITY COUNCIL
THIS AGREEMENT is made and entered into as of the date of last execution hereof by
and between SALT LAKE CITY CORPORATION, a municipal corporation of the State of Utah,
hereinafter "City", and 0, a , hereinafter"Consultant".
WITNESSETH:
WHEREAS, Consultant desires to provide certain auditing services for the Salt Lake
City Council, and
WHEREAS, City desires to engage Consultant for such services.
NOW, THEREFORE, in consideration of the promises and covenants hereinafter
contained, it is agreed by and between the parties hereto as follows:
1. Consultant agrees to provide for the City the auditing services described in
Exhibit "A", attached and incorporated by reference, for a period commencing as of the date of
execution of this Agreement and shall complete the final report within four months thereafter. All
financial commitments by the City shall be subject to the availability of funds approved by the City
Council and the limitations on future budget commitments provided under applicable Utah law,
including the Utah Constitution.

2. Said services shall consist of the services and requirements listed under Exhibit
"A", Scope of Work, attached and incorporated by reference.
3. For such services, Consultant shall be paid as specified under Exhibit "B", Price
Schedule, attached and incorporated by reference.
4. For such consideration, Consultant shall furnish all materials, supervision, labor
and equipment to complete the requirements of this Agreement.
5. Consultant, at its own cost, shall secure and maintain the following minimum
insurance coverage:
A. Worker's compensation and employer's liability insurance sufficient to
cover all of Consultant's employees pursuant to Utah State statutes. This requirement includes
Consultants who are doing business as an individual and/or as a sole proprietor as well as
corporations and partnerships. The certificate and policy shall provide that coverage thereunder shall
not be canceled or reduced without at least thirty(30) days prior written notice to the City.
B. Auditor shall carry professional liability insurance in an amount of not less
than $1,000,000 per claim. The certificate shall provide that coverage thereunder shall not be canceled
or modified without thirty(30) days prior written notice to the Agency.
C. Commercial automobile liability insurance that provides coverage for
owned, hired and non-owned automobiles, with the City named as an additional insured, in the minimum
amount of$1,000,000 per occurrence. The certificate and policy shall provide that coverage thereunder
shall not be canceled or modified without at least thirty (30) days written notice to the City.
D. All policies of insurance provided shall be issued by insurance companies
licensed to do business in the State of Utah and either:
(1) Rated with an A- or better rating in the most current edition of
Best's Key Rating Guide—Property-Casualty United States.
--OR--
(2) Listed in the United States Treasury Department's current Listing
of Approved Sureties(Department Circular 570, as amended).
E. Consultant shall furnish certificates of insurance, acceptable to the City,
verifying the foregoing concurrent with the execution hereof and thereafter as required.
F. In the event that governmental immunity limits are subsequently altered
by legislation or judicial opinion, the Consultant shall be required to provide a new certificate of
insurance within thirty(30) days of being notified thereof in writing by the City, certifying coverage in
compliance with the modified limits or, if no new limits are specified, in such an amount as may be
acceptable to the Salt Lake City Attorney's Office.

6. Consultant shall obey all laws, ordinances, regulations and rules of the federal,
state, county and municipal governments that may be applicable to its operations. Said laws include,
but are not limited to, the Equal Employment Opportunity laws, the Fair Labor Standards Act,
Occupational Safety& Health Administration(OSHA) laws, and the Americans with Disabilities Act
(ADA). Any violation of applicable law shall constitute a breach of this Agreement and Consultant shall
hold the City harmless from any and all liability arising out of, or in connection with, said violations
including any attorney's fees and costs incurred by the City as a result of such violation.
7. Either party may cancel this Agreement for any reason upon giving the other
party thirty(30) days prior written notice. Such notice shall be sent to the last known address of the
party to be notified.
8. The City may, without prejudice to any right or remedy, and without the
necessity of giving the thirty(30) day notice provided in Paragraph 7 above, terminate this Agreement
for cause in the event Consultant fails to fulfill, in a timely or satisfactory manner, any of the terms and
conditions set forth in this Agreement, and fails to cure any default after seven(7) days written notice
from the City of such default or breach.
9. If this Agreement is canceled or terminated as provided herein, City shall pay the
Consultant on the basis of actual services performed in accordance with the terms hereof.
10. Consultant, for itself, its successors and assigns, as part of the consideration
herefore, covenants that no person, solely on the grounds of race, color, national origin, age, sex,
religion or non job related disability, shall be excluded from participation in, denied the benefits of, or be
otherwise subject to discrimination in the furnishing of services hereunder, unless the characteristic is a
bona fide occupational qualification.
12. REPRESENTATION REGARDING ETHICAL STANDARDS FOR CITY
OFFICERS AND EMPLOYEES AND FORMER CITY OFFICERS AND EMPLOYEES.
Consultant represents that it has not: (1) provided an illegal gift or payoff to a City officer or employee

13. Consultant is not an employee of the City for any purpose whatsoever. The
Consultant is an independent Consultant at all times during the performance of the services specified
herein.

hereunder in good faith.
18. The parties agree that all provisions that are intended, directly or indirectly, by
their nature or by implication, to survive performance of the Consultant do survive the completion of
performance, termination or expiration of this Agreement.
19. This Agreement embodies the entire Agreement between the parties and shall not
be altered except in writing signed by both an authorized representative of Consultant and by the City's
Mayor or Mayor's designee. The terms of this Agreement shall supersede any additional or conflicting
terms or provisions that may be set forth or printed on Consultant's work plans, cost estimate forms,
receiving tickets, invoices, or any other related standard forms or documents of Consultant that may
subsequently be used to implement, record, or invoice services hereunder from time to time, even if
such standard forms or documents have been signed or initialed by a City representative. The intent of
the parties is that the terms of this Agreement shall prevail in any dispute between the terms of this
Agreement and the terms printed on any such standard forms or documents, and such standard forms or
documents shall not be considered written amendments of this Agreement.
20. This Agreement shall be enforced in and governed by the laws of the State of
Utah.

SCOPE OF WORK
MANAGEMENT AND PERFORMANCE AUDIT
OF FIRE DEPARTMENT
I. GENERAL
A. Consultant, if doing business under an assumed name, i.e. an Individual, Association,
Partnership, Corporation, or otherwise, shall be registered with the Utah State Division
of Corporations and Commercial Code.

B. Consultant shall assume full responsibility for damage to City property caused by
Consultant's employees or equipment as determined by designated City personnel.
C. Consultant shall be solely responsible for the safety of Consultant's employees and others
relative to Consultant's work, work procedures, material, equipment, transportation,
signage and related activities and equipment.
D. Consultant shall possess and keep in force all licenses, business permits and other permits
required to perform the services of this Agreement.
E. This is a non-exclusive agreement. The City may now or hereafter enter into agreements
with other consultants for management and performance audits.
F. Consultant shall notify the audit manager in the Council office of scheduled meetings
with the Administration.
H. RESPONSIBILITIES OF THE CONSULTANT
The responsibilities of the Consultant include, but shall not be limited to, the following:
A. Compare Salt Lake City's Fire Department to at least 10 comparable cities.
B. Review current operations including facilities, equipment, and organization. Determine
whether existing facilities are adequate or whether there are opportunities for
implementing cost-saving measures or consolidating functions or facilities. Analyze
opportunities to increase productivity and efficiency through the purchase of additional
equipment or the replacement of old equipment.

C. Analyze the Department's response time performance in fire and emergency medical
services. Provide recommendations for any changes in current station locations and
apparatus placement.
D. Assess whether Salt Lake City's Fire Department provides the same range of services as
other similar size fire departments.
E. Identify services and programs that are generally considered core functions and those
that are additional programs that the Fire Department provides as added services to the
public.
F. Review non-core services such as community education programs (including fire
education in elementary schools) and compare to regional peer programs.
G. Assess and compare best practices in use of technical advances in emergency response to
enhance the City's current fire and emergency medical services.
H. Assess and evaluate the costs and benefits of the Fire Department's current deployment
of four person crews based upon accepted industry practice and comparison to regional
peer operations.
I. Evaluate the Department's daily staffing, overtime, constant staffing, and related
procedures compared to other fire departments and best practices. Develop and propose
alternatives.
J. Review and analyze staffing levels of non-safety and support staff positions. Provide
recommendations regarding staffing levels and practices necessary to support
recommended operations.
K. Evaluate training, skills maintenance, certification, technical rescue, and other capabilities
compared to best practices.
L. Review the Department's HazMat delivery capability compared to best practices.
M. Analyze the Department's incident management and control procedure and practices
compared to best practices.
N. Evaluate the adequacy of the Department's mutual aid agreements compared to best
practices.
O. Compare the Fire Department's physical fitness maintenance program with best practices
and evaluate whether standards and requirements are adequate to ensure that fitness for
duty is maintained until retirement.
P. Review the call taking, call handling and dispatching procedures and practices including
protocols and quality assurance compared to best practices.
Q. Review the fire investigation program compared to other cities and best practices.
Determine whether opportunities exist to increase effectiveness through shared service
arrangements or other efficiencies.

R. Analyze code enforcement and inspection program. Determine workloads and staffing
needs. Identify opportunities for further collaboration or coordination with other City
permit issuance functions; identify opportunities to collaborate or eliminate duplication in
building code and fire code enforcement, permit issuance or plan evaluation. Compare
Salt Lake City's handing of plan review, design issues, and post fire computer modeling
with best practices. Address whether other cities employ fire protection engineers.
S. Determine whether fees for services, such as fire inspections, are charged in other
communities; the pros and cons associated with the approaches; and whether it might be
appropriate to consider fees for service. Determine the extent to which Salt Lake City
assesses fees by City ordinance and the extent to which those services are conducted and
fees charged.
T. Assess the Fire Department's readiness for a large-scale emergency. Make
recommendations to further prepare. Assess the City's current emergency management
program and determine whether there is adequate coordination and interaction with the
Fire Department.
U. Analyze the Department's communication capabilities with other Salt Lake City
departments and with other state and local agencies. Assess long-term plans for
communication systems.
V. Assess the Department's current Emergency Operations Center and the Department's
plans for relocating the Emergency Operations Center. Include a review of the
possibility to relocate the fire administration function to the new Emergency Operations
Center site. Comment on the potential joint tenancy with the Red Cross.
W. Evaluate whether the City's current development patterns allow for appropriate,
necessary and timely emergency access and whether there are ordinance or policy
changes that the City should consider to ensure that the City considers access for
emergency equipment as part of all requests for development, including that in the
foothills.
X. Evaluate coordination of the Transportation Division with the Fire Department before
designing traffic calming measures to determine whether the measures impact emergency
response. Determine whether the City's recent traffic calming measures accommodate
emergency response concerns in addition to traffic management needs.
Y. Identify pros and cons of the City providing ambulance or other services.
Z. Identify policy and budget considerations that the Council may want to be aware of.
Ill. DELIVERABLES
Consultant shall provide the City Council with the following:
A. Each audit report shall include an executive summary, background, observations,
recommendations and management's response unless management declines to provide a
written response.

B. Provide a draft audit report to the Fire Chief and to the Council's audit subcommittee.
C. The Consultant shall meet with the City Council in a public meeting to make an oral
presentation of the final audit report and to respond to questions from the Council.
D. Provide twenty-five(25) copies of the final audit report.
E. Upon written authorization of the City Council, Consultant shall give duly authorized
representatives of the City access to, and the right to examine, any notes, working
papers, statements and other documentation prepared in connection with Consultant's
performance. Work papers shall be retained for a period of three years after the
completion of the audit.
IV. DISCLOSURE OF CITY RECORDS
Since the City shall own the documents generated by the Consultant pursuant to this Agreement,
the Consultant agrees that it shall not, without written approval by the City, disclose publicly
said records. Consultant understands that the information obtained in the performance of this
Agreement is confidential and may be shared with employees of the City or others only on a
need to know basis.

INFORMATION& REQUIREMENTS
I. OBJECTIVE. Salt Lake City Corporation(the "City"), is soliciting competitive sealed
proposals from qualified consultants to conduct management and operational reviews on an as-
requested basis for the City Council. This Request for Proposal describes Salt Lake City, the
purpose of the reviews, the process for determining a scope of services and a budget for each
review, proposal and contract requirements and related information.
II. BACKGROUND.
Salt Lake City is Utah's capital city, the county seat of Salt Lake County, and the largest city in
the four county Wasatch Front metropolitan area. The City's population increases daily from
approximately 182,000 to nearly 300,000 as workers commute to jobs located within the City's
jurisdictional limits. The City operates under the Mayor-Council form of government,with a
full-time Mayor elected at-large and seven part-time Council Members representing specific
geographic districts. More information can be found on the City's web site at www.slcgov.com.
The Salt Lake City Council exercises its oversight role of the City administration by conducting
independent management and operational reviews of City programs and functions. The reviews
are conducted by auditing or consulting firms selected by the Council. The City Council's
objective of the reviews is to address the issue of how effectively services are provided and how
well desired results and benefits are being achieved. The purpose of the review is to assess
performance, to identify opportunities for improvement, and to develop specific
recommendations for action. A primary focus of the services is to examine the program or
function against best demonstrated practices and national standards. This comparative approach
is intended to identify opportunities to increase service levels or operational efficiencies.
The Council may have contracts with more than one auditing or consulting firm. The Council
identifies a list of programs or functions for potential audit. These potential audits are
traditionally ranked by individual Council Members and prioritized by a Council subcommittee.
Specific audit scope and objective statements are identified and a budget is developed for each
audit. The audits are not extensive or all-encompassing but are very limited-scope studies with
rather small budgets. Based on the particular experience of the consultant and based on other
factors such as availability of the consultant to provide timely services, the Council
subcommittee selects a consultant from those firms from which the Council has agreements for
audit services. The entire Council is briefed on the selection by the Council subcommittee. The
Council Chair will issue a letter to the consultant or consultants outlining the scope and
objectives of the audit and authorizing a budget amount.
III. GENERAL INFORMATION
Offerors should review the required insurance coverage that will be part of the resulting
contract(s). Insurance information is provided under Paragraph 5 of the Sample
Agreement. Proposed pricing must include associated insurance costs.
Offerors are advised that the selected consultant for this project will not be authorized to
publicize or refer to the 2002 Winter Olympic Games, the year 2002 in an Olympic
context, or imply any connection between itself and the Olympic Games, in any
advertising, marketing and other materials.

Instructions: When preparing proposals, reply to each of the following proposal content&
evaluation criteria in the order listed. Please restate each numbered point listed below followed
by your response in full, narrative sentences and provide any requested materials:
QUALIFICATIONS
A. A statement of the firm's experience and qualifications to meet the requirements of the
City as outlined herein including the names, titles and telephone numbers of previous
clients who can speak to the firm's ability to perform accordingly.
B. Qualifications of staff who would be assigned to this project. Please attach resumes.

identify any costs for such services,products, knowledge, or skills. If there is a better
way for the City to package or define the services that it has requested herein in order to
obtain better pricing and/or service by making it easier for the selected consultant, please
explain what modifications would be necessary to achieve such better pricing and/or
service.
III. PROPOSED FEES
Each audit assignment will be requested in writing from the Chair of the City Council. The
written request will outline the scope and objectives of the review and state an authorized
maximum fee amount for each review.

environment. Please list measures such as alternative fuel vehicles, recycling measures

AWARD BY WRITTEN AGREEMENT
The offeror selected to provide the services/products shall be required to enter into a written
agreement in substantially the form as shown in the attached SAMPLE AGREEMENT
(ATTACHMENT 3) which shall be the basic form used to develop the final agreement.
• Signature on the Proposal Cover Sheet acknowledges that the offeror is willing to enter into
the agreement if awarded the contract. Offerors are advised to read thoroughly the Sample
Agreement as the selected offeror will be required to comply with its requirements.
• If offeror has any exceptions to the Sample Agreement, the procedures stated under
Paragraph IV, EXCEPTIONS, of this section must be followed.
• Offerors should pay particular attention to insurance coverage requirements specified
in Paragraph 5 of the Sample Agreement. The selected consultant will be required to
provide insurance certificates at the time of notification of conditional award.
II. PREPARATION OF PROPOSALS
A. Failure to Read. Failure to read the Request for Proposal and these instructions will be
at the offeror's own risk.
B. Cost Of Developing Proposals. All costs related to the preparation of the proposals and
any related activities are the sole responsibility of the offeror. The City assumes no
liability for any costs incurred by offerors throughout the entire selection process.
III. PROPOSAL INFORMATION
A. Discussions With Offerors. The City may conduct discussions with offerors who submit
proposals determined to be reasonably susceptible of being selected for award.
However, proposals may be accepted without such discussions, at the City's option.
B. Equal Opportunity. The City will make every effort to ensure that all offerors are treated
fairly and equally throughout the entire advertisement, review and selection process.

The procedures established herein are designed to give all parties reasonable access to
the same basic information.
C. Proposal Ownership. All proposals, including attachments, supplementary materials,
addenda, etc., shall become the property of the City and will not be returned to the
offeror.
D. Rejection Of Proposals.
• The City reserves the right to reject any or all proposals received. Furthermore, the
City shall have the right to waive any informality or technicality in proposals received
when in the best interest of the City.
• No proposal shall be accepted from, or contract awarded to, any person, firm or
corporation that is in arrears to the City,upon debt or contract or that is a defaulter,
as surety or otherwise, upon any obligation to the City, or that may be deemed
irresponsible or unreliable by the Purchasing, Contracts&Property Management
Division. Offerors may be required to submit satisfactory evidence that they have the
necessary financial resources to perform and complete the work outlined in this RFP.
E. Failure To Submit A Proposal. Failure to submit a proposal(or to advise the City
Purchasing, Contracts&Property Management Division that future Requests for
Proposal are desired)may result in the removal of your firm from the prospective
offerors list.
IV. EXCEPTIONS TO PROPOSAL & SAMPLE AGREEMENT
If offeror takes exception to any term or condition set forth in this proposal and/or the Sample
Agreement and any of its Exhibits and Attachments, said exceptions must be clearly identified in
the response to this RFP. Exceptions or deviations to any of the terms and conditions must
not be added to the proposal pages but must be submitted in a separate document
accompanying offeror's proposal identified as "Exceptions." Such exceptions shall be
considered in the evaluation and the award processes. The City shall be the sole determiner of
the acceptability of any exception.
Should the City omit anything from this Request for Proposal which is necessary for a clear
understanding of the work, or should it appear that instructions are in conflict, then the offeror
shall secure written instructions from the Purchasing, Contracts&Property Management
Division at least forty-eight (48)hours prior to the time and date of the proposal opening shown
above.
V. CONFIDENTIALITY
All responses, inquiries, and correspondence relating to this RFP and all reports, charts, displays,
schedules, exhibits, and other documentation produced by the offeror that is submitted to the
City, as part of the proposal or otherwise, shall become the property of the City when received
by the City and may be considered public information under applicable law. The City is subject

to the disclosure requirements of the Government Records Access and Management Act, Title
63, Chapter 2, Utah Code Annotated. The City generally considers proposals and all
accompanying material to be public and subject to disclosure. Any material considered by the
offeror to be proprietary must be accompanied by a written claim of confidentiality and a
concise written statement of reasons supporting the claim. Blanket claims that the entire
RFP is confidential will be denied. The City cannot guarantee that any information will be
held confidential. Under Section 63-2-304 of the Government Records Access and Management
Act, if the offeror makes a claim of confidentiality, the City, upon receipt of a request for
disclosure, will determine whether the material should be classified as public or protected, and
will notify the offeror of such determination. The offeror is entitled under the Government
Records Access and Management Act to appeal an adverse determination. The City is not
obligated to notify the offeror of a request, and will not consider a claim of confidentiality,
unless the offeror's claim of confidentiality is made in a timely basis and in accordance
with the Government Records Access and Management Act.
VI. REPRESENTATION REGARDING ETHICAL STANDARDS FOR CITY OFFICERS
AND EMPLOYEES AND FORMER CITY OFFICERS AND EMPLOYEES
The offeror represents that it has not: (1)provided an illegal gift or payoff to a City officer or
employee or former City officer or employee, or his or her relative or business entity; (2)
retained any person to solicit or secure this contract upon an agreement or understanding for a
commission, percentage, brokerage or contingent fee, other than bona fide employees or bona
fide commercial selling agencies for the purpose of securing business; (3)knowingly breached
any of the ethical standards set forth in the City's conflict of interest ordinance, Chapter 2.44,
Salt Lake City Code; or(4)knowingly influenced, and hereby promises that it will not
knowingly influence, a City officer or employee or former City officer or employee to breach
any of the ethical standards set forth in the City's conflict of interest ordinance, Chapter 2.44,
Salt Lake City Code.
VII. GENERAL INFORMATION
A. The City has developed a process by which vendors can receive payments and remittance
advices electronically. The electronic process will (1) allow vendors to receive collected
funds the day following the City's weekly "check write," and (2)will provide an
electronic remittance advice on the day of the "check write." The remittance advice will
be sent via e-mail through the internet to as many as three separate e-mail addresses.
The remittance advice will contain exactly the same information that would have been
included on a check stub. Also available is a vendor web page that allows a vendor, with
proper user name and password, to see its history of payments from the City. The City
encourages offerors to accept electronic payments using this process.

SAMPLE AGREEMENT
SPECIAL AUDIT SERVICES
FOR THE SALT LAKE CITY COUNCIL
THIS AGREEMENT is made and entered into as of the date of last execution hereof by
and between SALT LAKE CITY CORPORATION, a municipal corporation of the State of Utah,
hereinafter "City", and 0, a_, hereinafter"Consultant".
WITNESSETH:
WHEREAS, Consultant desires to provide certain_for the City's SALT LAKE CITY
COUNCIL; and
WHEREAS, City desires to engage Consultant for such services.
NOW, THEREFORE, in consideration of the promises and covenants hereinafter
contained, it is agreed by and between the parties hereto as follows:
1. Consultant agrees to provide for the City auditing services described in Exhibit
"A", attached and incorporated by reference, for a period of two (2)years commencing as of the date of
execution of this Agreement. City may extend this Agreement for three(3) additional 1-year time
periods, renewable on an annual basis, under the same terms and conditions. Notice of time extensions
shall be in writing served upon the Consultant by regular mail at least thirty(30) days prior to the
expiration of the original term of this Agreement, or any current extension, in order for such extension
to be effective. After all annual extensions have been exercised City shall have the right to extend this
Agreement on a month-to-month basis for a period not to exceed four(4) months. Notice of the
month-to-month extension shall be in writing served upon the Consultant by regular mail at least ten
(10) days prior to the expiration of the last annual extension, in order for such extension to be effective.
City shall give at least five (5) days written notice of cancellation of the month-to-month extension.
This is a non-exclusive Agreement and the City reserves the right to acquire the services, at its

discretion, from other sources during the term of this Agreement. All financial commitments by the City
shall be subject to the availability of funds approved by the City Council and the limitations on future
budget commitments provided under applicable Utah law, including the Utah Constitution.
•
2. Said services shall consist of the services and requirements listed under Exhibit
"A", Scope of Work, attached and incorporated by reference.
3. For such services, Consultant shall be paid as specified under Exhibit "B", Price
Schedule, attached and incorporated by reference.
4. For such consideration, Consultant shall furnish all materials, supervision, labor
and equipment to complete the requirements of this Agreement.
5. Consultant, at its own cost, shall secure and maintain the following minimum
insurance coverage:
A. Worker's compensation and employer's liability insurance sufficient to
cover all of Consultant's employees pursuant to Utah State statutes. This requirement includes
Consultants who are doing business as an individual and/or as a sole proprietor as well as
corporations and partnerships. The certificate and policy shall provide that coverage thereunder shall
not be canceled or reduced without at least thirty (30) days prior written notice to the City.
B. Professional liability insurance in an amount of not less than $1,000,000.
The certificate and policy shall provide that coverage thereunder shall not be canceled or modified
without thirty (30) days prior written notice to the City.
C. Commercial automobile liability insurance that provides coverage for
owned, hired and non-owned automobiles, with the City named as an additional insured, in the minimum
amount of$1,000,000 per occurrence. The certificate and policy shall provide that coverage thereunder
shall not be canceled or modified without at least thirty(30) days written notice to the City.
D. All policies of insurance provided shall be issued by insurance companies
licensed to do business in the State of Utah and either:
(1) Rated with an A- or better rating in the most current edition of
Best's Key Rating Guide—Property-Casualty United States.
--OR--
(2) Listed in the United States Treasury Department's current Listing
of Approved Sureties(Department Circular 570, as amended).
E. Consultant shall furnish certificates of insurance, acceptable to the City,
verifying the foregoing concurrent with the execution hereof and thereafter as required.
F. In the event that governmental immunity limits are subsequently altered
by legislation or judicial opinion, the Consultant shall be required to provide a new certificate of
insurance within thirty (30) days of being notified thereof in writing by the City, certifying coverage in

compliance with the modified limits or, if no new limits are specified, in such an amount as may be
acceptable to the Salt Lake City Attorney's Office.
6. Consultant shall obey all laws, ordinances, regulations and rules of the federal,
state, county and municipal governments that may be applicable to its operations. Said laws include,
but are not limited to, the Equal Employment Opportunity laws, the Fair Labor Standards Act,
Occupational Safety&Health Administration(OSHA)laws, and the Americans with Disabilities Act
(ADA). Any violation of applicable law shall constitute a breach of this Agreement and Consultant shall
hold the City harmless from any and all liability arising out of, or in connection with, said violations
including any attorney's fees and costs incurred by the City as a result of such violation.
7. Either party may cancel this Agreement for any reason upon giving the other
party thirty(30) days prior written notice. Such notice shall be sent to the last known address of the
party to be notified.
8. The City may, without prejudice to any right or remedy, and without the
necessity of giving the thirty(30) day notice provided in Paragraph 7 above, terminate this Agreement
for cause in the event Consultant fails to fulfill, in a timely or satisfactory manner, any of the terms and
conditions set forth in this Agreement, and fails to cure any default after seven(7) days written notice
from the City of such default or breach.
9. If this Agreement is canceled or terminated as provided herein, City shall pay the
Consultant on the basis of actual services satisfactorily performed as calculated by the City.
10. Consultant, for itself, its successors and assigns, as part of the consideration
herefore, covenants that no person, solely on the grounds of race, color, national origin, age, sex,
religion or non job related disability, shall be excluded from participation in, denied the benefits of, or be
otherwise subject to discrimination in the furnishing of services hereunder, unless the characteristic is a
bona fide occupational qualification.

ordinance, Chapter 2.44, Salt Lake City Code.
12. Consultant shall not publicize or refer to the 2002 Winter Olympic Games, the
year 2002 or Salt Lake City in an Olympic context, or imply any connection between itself and the
Olympic Games, in any advertising, marketing and other materials.

13. Consultant is not an employee of the City for any purpose whatsoever. The
Consultant is an independent Consultant at all times during the performance of the services specified
herein.

15. This Agreement shall not be assigned by either party without the prior written
consent of the other party.
16. Consultant's obligations are solely to the City and the City's obligations are solely
to Consultant. This Agreement shall confer no third party rights whatsoever.
17. This Agreement embodies the entire Agreement between the parties and shall not
be altered except in writing signed by both an authorized representative of Consultant and by the City's
Mayor or Mayor's designee. The terms of this Agreement shall supersede any additional or conflicting
terms or provisions that may be set forth or printed on Consultant's work plans, cost estimate forms,
receiving tickets, invoices, or any other related standard forms or documents of Consultant that may
subsequently be used to implement, record, or invoice services hereunder from time to time, even if
such standard forms or documents have been signed or initialed by a City representative. The intent of
the parties is that the terms of this Agreement shall prevail in any dispute between the terms of this
Agreement and the terms printed on any such standard forms or documents, and such standard forms or
documents shall not be considered written amendments of this Agreement.
18. This Agreement shall be enforced in and governed by the laws of the State of
Utah.

EXHIBIT "A"
SCOPE OF WORK
SPECIAL AUDIT SERVICES
I. GENERAL
A. Consultant, if doing business under an assumed name, i.e. an Individual, Association,
Partnership, Corporation, or otherwise, shall be registered with the Utah State Division
of Corporations and Commercial Code.

B. Consultant shall assume full responsibility for damage to City property caused by
Consultant's employees or equipment as determined by designated City personnel.
C. Consultant shall be solely responsible for the safety of Consultant's employees and others
relative to Consultant's work, work procedures, material, equipment, transportation,
signage and related activities and equipment.
D. Consultant shall possess and keep in force all licenses, business permits and other permits
required to perform the services of this Agreement.
E. No guarantee of the actual service requirement is implied or expressed by this
Agreement. Service requirements shall be determined by actual need.
F. This is a non-exclusive agreement. The City may now or hereafter enter into agreements
with other Consultants to provide special audit services. City employees may elect to
place orders with one or more Consultants under agreement with selection made on the
basis of price, location, hours of operation, and/or availability of needed services.
11. RESPONSIBILITIES OF THE CONSULTANT
The responsibilities of the Consultant include, but shall not be limited to, the following.
A. When requested, submit a one to two-page letter outlining how the firm would approach
a particular topic being considered for audit and the fee the firm would charge for the
project. Any costs associated with preparing these letters will be at the firm's expense
and will not be reimbursed by the City Council Office.
B. Conduct management and operational reviews as requested by the City Council.
Requests will be in writing from the Chair of the City Council. The written request will
outline the scope and objective of the review and authorized maximum fee amount.

C. In order to determine best practices for each audit assignment, Consultant shall compare
Salt Lake City's program or function to at least 10 similar cities.
D. Meet with the Council's audit subcommittee periodically to review audit progress or to
discuss draft audit reports.
E. Do not exceed the authorized maximum fee amounts for any individual audit assignment
unless written authorization from the Council Chair expands the scope of work and
authorizes additional fees.
III. DELIVERABLES
Consultant shall provide the City with the following:
A. Provide draft audit reports to the City managers responsible for the program audited and
to the Council's audit subcommittee. Attend meetings of the Council's audit
subcommittee when audit reports are discussed to make short oral presentations or
respond the committee members' questions.
B. Twenty-five(25) copies of final audit reports.
C. Each audit report shall include an executive summary, background, observations,
recommendations, and management's response unless management declines to provide a
written response.
D. Consultant shall meet with the City Council to present each audit report.
E. If authorized in writing by the Council Chair, Consultant shall give duly authorized
representatives of the City access to, and the right to examine, any notes, working
papers, statements and other documentation prepared in connection with Consultant's
performance. Work papers shall be retained for a period of three years after the
completion of the audit.
IV. DISCLOSURE OF CITY RECORDS
Since the City shall own the audit reports generated by the Consultant pursuant to this
Agreement, the Consultant agrees that it shall not, without written approval by the City, disclose
publicly said reports. Consultant understands that the information obtained in the performance of
this Agreement is confidential and may be shared with employees of the City or others only on a
need to know basis.
V. PRODUCTS/SERVICES FOR OTHER CITY DEPARTMENTS
With the written approval of the City Council Division the Consultant may respond to calls from
other City departments to provide the specified services. All services shall be provided at the
rates stated in Exhibit `B". Invoices shall be sent directly to the department originating the
request.

EXHIBIT "B"
PRICE SCHEDULE
SPECIAL AUDIT SERVICES
GENERAL
A. Prices stated include all costs associated with the performance of the services specified,
including materials, supervision, labor, transportation, delivery, and related costs. No
other charges shall be allowed.
B. City is exempt from sales, use and federal excise taxes on these products and/or services.
Exemption certificates shall be furnished upon request.
C. Prices and/or discounts stated shall be firm for the initial 2-year term of this Agreement.
Requests for price adjustment thereafter shall follow requirements specified in Paragraph
III, Option Renewal Period Price Adjustment.
II. PRICING FEES

III. OPTION RENEWAL PERIOD PRICE ADJUSTMENT.
Prices stated are firm for the initial 2-year term of this Agreement. If this Agreement is extended
for additional option periods price adjustments may be made, however, any request for price
adjustment shall be made prior to the beginning date of the contract option renewal. Consultant
shall calculate and make a request of the City that the prices for the next renewal year be
increased by the lesser of three percent (3%), or the same percentage increase, if any, in the
latest published Consumer Price Index, All Urban Consumers (CPI-U), "US City Average"
published by the Bureau of Labor Statistics of the US Federal Government. If publication of

said CPI index should cease, such percentage increase shall be determined by reference to a
similar index, or as agreed upon by the parties.
IV. INVOICING AND PAYMENT •
City shall make payment to Consultant for all services performed by Consultant pursuant to this
Agreement. Consultant shall submit a written invoice, in duplicate, for services rendered and
City shall pay the invoiced fee within thirty(30) days after receipt of the invoice by City.

1. REQUEST FOR PROPOSAL
The Redevelopment g , „f c It T k Cit ("" ") Salt Lake City Council,acting in its
capacity as the Redevelopment Agency Board of Directors, is requesting proposals from
qualified consultants(hereinafter same-"Consultant")for management auditing services.
The Salt Lake City Council hereinafter referred to as "Board of Directors."
2. GENERAL INFORMATION ABOUT THE AGENCY.
The Redevelopment Agency of Salt Lake City(Agency)was incorporated in June 10, 1969. The
governing statutes for redevelopment agencies can be found in the Utah State Code, as amended,
1953, Title 17B-4, Chapter 4. The Agency currently has six active project areas located
throughout the City. The project areas are primarily focused in the commercial or mixed-use
districts of the City. The elected members of the Salt Lake City Council are the Board of
Directors of the Agency. The Board of Directors has adopted by-laws which describe'the duties
and responsibilities of the Board that are primarily policy and the designation of the Salt Lake
City Mayor to act as the Chief Administrative Officer handing the day-to-day administration of
the Agency.
Current Agency staff consists of the following eight positions: Executive Director,Deputy
Director, Senior Project Manager,two Project Coordinators, Property Manager, Office
Facilitator, and Office Technician. For the past three fiscal years,the Agency's Annual
Implementation Budget has ranged from$30,500,000 to $48,000,000. Approximately one-third
of the Agency's budget is used to retire debt.
The Redevelopment Agency of Salt Lake City exists to improve blighted areas of Salt Lake City,
encourage economic development of Salt Lake City, and encourage the development of housing
for low and moderate income households within Salt Lake City. Some of the tools available to
the Redevelopment Agency include:
• Property acquisition, clearance,re-planning, sale or redevelopment
• Planning, financing and development of public improvements
• Providing management support and financing for projects which will improve blighted
areas
• Gap financing in the form of loans, grants and equity participation to encourage private
investment
• Relocation assistance and business retention assistance to businesses

3. SCOPE OF SERVICES.
The selected Consultant shall provide a written management audit reports which will provide
options and recommendations to help the Agency achieve its the Agency's goals and objectives
including
,the costs, benefits and risks of options to close any gap between where
the Agency is and where it needs to bet and 3)-under-standing-how-the-Ageney-ean to ensure the
City and its citizens are receiving good value for its investment.
Specifically, we are proposing that the management audit provide information about whether or
not the Agency(1) is cost effective; (2)has an effective management structure including staffing
and operations when compared to other similar agencies; (3)is responsive to the needs of
customers(which includes city departments and the community),management and staff,the
administration, and the board; 4)is responding pro-actively and reactively to changes in those
needs; and 5) is encouraging informed initiatives from staff. The management audit should
identify opportunities to improve the effectiveness,efficiency and responsiveness of the staff by
developing specific recommendations to increase service levels and/or operational efficiencies.
The audit should also comment on the project analyses prepared by staff members compared with
I other city Salt Lake City departments and best practices.
The management audit should consider the following in preparing its report:
• The strategic plans prepared for accomplishment of the redevelopment plans established
for the project areas, general administration, and housing development.
• The current organizational structur .
• Human resource management including delegation and methods of assigning
responsibility and accountability for accomplishing Agency goals.
• The budgeting process,purchasing system, accounting systems, and cash management
controls.
• The Agency's statutory and legal obligations including the role of the Board and
Administration in effective and efficient management.
• Communications and feedback loops at all levels, including board,administration,
management, staff, and customers.
• A discussion of the long-term organizational structure given current statutory
requirements.
• Other recommendations which may come about as a result of the consultant's work.
The Utah Redevelopment Agencies Act requires that project areas be created for a specific period
of time known as the"sunset"provision. The report should discuss potential effects of the sunset
provisions on the management structure and staffing.
The Agency's Board of Directors has formed a committee to assist the Consultant in research and
to provide guidance during the data gathering phase of this project. The committee is made up of
representatives from the Agency's Board of Directors,the Administration,and-a member of the
Redevelopment Advisory Committee, and the Audit Coordinator. The Consultant will be
expected to meet with the committee from time to time to bring them up to date on the status of
the audit and to use the committee as a source of information and direction.
Additional services may be requested from the Consultant. As part of the Fee Proposal,
Consultant shall provide hourly rates for personnel assigned to work on the Management Audit.
Upon completion of the final report,the Consultant shall provide 25 copies. In-addition-to

presenter the results of the audit during-at-least-one public meeting to be determined by
the Agency Board of Directors.
4. QUALIFICATIONS.
The selected Consultant shall provide as part of its response to this Request for Proposals a
statement detailing certifications and qualifications to complete the management audit as
requested.
5. WRITTEN AGREEMENT REQUIRED.
I The selected Consultant shall enter into a written agreement with the Agency-Board of Directors,
to provide the services described in the Scope of Services. The Scope of Services may be
modified by the written agreement. The form of written Consulting Services Agreement
I proposed by the Agency is attached as Exhibit"A" and incorporated herein by reference.
Consultants shall include as a part of their proposal a list of any changes Consultant proposes to
the draft agreement.
6. TERM OF AGREEMENT.
The Agreement for Consulting Services with the selected Consultant shall expire upon
completion of e S r f e b o f t M h i 5,200 one year from the effective
date of the agreement.
7. SCHEDULE.
The selected Consultant shall complete the audit and issue a final report
n or before March 14,2004.
8. FEES AND CHARGES.
-Consultants shall provide ' a fee proposal in the form attached as
Exhibit "B" stating the proposed fixed-not-to-exceed fee to complete the Scope of Services.
9. CONTENT OF PROPOSAL.
Consultants shall provide the following information in their Proposal for evaluation by the
Agency Selection Committee and Board of Directors:
• A list and description of projects which the Consultant has provided similar professional
services; the names of the personnel of the Consultant or Consultant's proposed
subconsultants who performed the work;a discussion of whether or not the projects were
completed on time and within budget.
• A list of the subconsultants proposed to be used by the Consultant in the performance of
the services described in the Scope of Services.
• Experience and past performance of the Consultant and the Consultant's proposed
subconsultants relating to redevelopment agencies.
• Names and qualifications(including professional training,licenses,experience,awards,
etc.)of personnel of Consultant and Consultant's proposed subconsultants for the
services described in the Scope of Services,the time commitments of each,the role of
each in the performance of the Scope of Services.
• A statement of the Consultant's proposed approach to managing the services described in
the Scope of Services, including a discussion of the Consultant's schedule for the
performance of services.
• A not-to-exceed fee proposal, estimated total hours, and hourly rates of individuals to be
assigned to the project.

• The Consultant's proposed approach to managing the services described in the Scope of
Services and the proposed time schedule to complete the audit;

11. SUBMISSION OF PROPOSAL AND DUE DATE.
In order to be considered for award, Consultants submitting a proposal shall submit eight(8)
copies of the written proposal including and-the fee proposedal to the Salt Lake City Council
Offices,451 South State Street, Room 304, Salt Lake City,Utah 84111,prior to 4:30 p.m. on
2003. The written proposal and-the-fee-proposal-shall be signed
in ink by an officer or employee of the Consultant authorized to bind the Consultant
contractually.

Proposals which are not received in the Agency offices City Council Office prior to the time and

12. ORAL PRESENTATION.
At the option of the Agency, Selection Committee or Board of Directors,the Consultants may be
requested to meet at the Consultant's expense with the Selection Committee for an oral

13. WITHDRAWAL OF PROPOSALS.
Proposals shall not be withdrawn and shall remain open for acceptance by the Agency for a

form of written Addenda. Copies of any such written Addenda issued by the Agency shall be
available for review by any interested party at the Agency's offices City Council Office prior to
the submission date.
16. RIGHT TO REJECT/WAIVE INFORMALITY OR IRREGULARITY.
The Agency Board of Directors, reserves the right to reject any and all proposals and to waive

Fourth Quarter
The First Time Homebuyer Program, which utilizes HOME, CDBG, Renter Rehab,
Housing Trust Fund,Utah State Funding and Bank Partners funding that includes Multi-Bank
Pool and Wells Fargo Bank,has placed 243 families into their first home. This fiscal year,
twenty-eight homes have been rehabilitated and two newly constructed. At the end of this
quarter, five First Time Homebuyer homes are in the process of rehabilitation, ten families were
placed in new homes, and four additional homes are ready for sale. Construction has begun on
the nine homes on Pugsley Street and the project is proceeding well.

CDBG funds include $67,733.00 in lead grants for this fiscal year.
Volume continues to be weak in both the rehab and first time homebuyer programs due to
the broad based weakness in the national economy. Clients are living a marginal financial
existence and many are reluctant to incur additional debt during uncertain times.
This year, the application window for the first time home buyer program has been left open on an
on-going basis. Staff has provided program information to an additional 60 church groups, a
display at the Care Fair and at the Greenbacks Conference for Single Women. In addition,
outreach efforts now include follow up of applications that have been requested by individuals.
Mailing outreach has generated nearly 40 potential projects and the Housing Division continues
to receive a few applications per month.
STATUS OF CURRENTLY FUNDED PROJECTS:
Pugsley West and Pugsley North Subdivision
The Pugsley West and North Subdivisions, located at approximately 400 West, 500-
600 North,was bid on August 9, 2002 for development of 9 single-family homes. Of these nine,
four will be sold to First Time Homebuyers. Housing and Neighborhood Development finalized
the land acquisition from the Salt Lake City Redevelopment Agency in December and
construction has begun. The homes located on Lots 3, 4 and 5 are nearing completion and have
had carpet installed. It is anticipated that the entire project will be complete by late August. This
is a joint venture between the City and Neighborhood Housing Services.
The Sherwood Place Subdivision has been bid and construction will begin in early
spring or summer after Pugsley is completed. Property continues to be monitored for cleanliness
and security.
The Phoenix Circle Subdivision status remains unchanged.

Please include the type of service or work performed in your grant, the unit of measure, and units
completed to date.
1. Description of service performed: The mission of Utah Nonprofit Housing Corporation is
to provide decent, safe, and affordable housing for low and very low income individuals and
families by: 1. Identifying critical needs for low and very low income housing in neighborhoods;
2. Exploring innovative ways to provide housing at the most affordable cost for those in target
populations; 3. Educating the community on issues related to affordable housing and advocating
for these in partnership with other community based groups; 4. Creating and promoting
partnerships with planners, developers, and potential public and private sponsors; 5. Being
available as a consultant and role model for other housing groups; 6. Assuring delivery of
appropriate social services by community agencies-.
2. Number of clients served: 1649 persons total, of which 1288 live within Salt Lake City
limits.
3. Areas of the city served: Of the 34 existing affordable housing projects owned wholly or
in part by Utah Nonprofit Housing Corporation, and its affiliated corporations, and shown on the
accompanying charts, 15 projects are in Salt Lake City. These are: The Sedona, Escalante Park I,
II & III, Rio Grande Hotel, Lowell Apartments, Riverwood Cove, Goshen Street, Jacob
Apaitiuents, Ivy House, Sego House, Villa Maria, Multi-Ethnic Highrise, Hamilton Place,
Preston Place, and Glendale Senior Housing. These projects in Salt Lake City total 739 units.

projected completion date, etc.: During the year(July 1, 2002 - June 30, 2003), Utah Nonprofit
Housing Corporation(UNPHC) along with its affiliated non-profit housing corporations and Low
Income Housing Tax Credit(LIHTC)partnerships,provided within the Salt Lake City limits 739
affordable units as follows: 524 units of elderly housing; 124 units of family housing; 40 units
targeted for homeless single women; 49 units targeted for single men; 2 units for homeless
HIV/AIDS persons. Outside Salt Lake City we have 234 units in Salt Lake County: 170 elderly,
42 for homeless persons, 12 for physically disabled, and 10 single-family rent-to-own homes.
While most of the 2002-2003 CDBG operating year's development projects are located in
cities outside Salt Lake City, the benefit to Salt Lake City is tangible. Affordable housing in
adjacent areas, including the unincorporated parts of Salt Lake County helps reduce the demand
on existing facilities within Salt Lake City proper. UNPHC remains important within Salt Lake
City as we own and operate 739 units of affordable housing within the Salt Lake City limits,
providing housing to more than 1000 low income persons. UNPHC strongly desires additional
projects with Salt Lake City Corporation, and always pursues any proposed opportunities to
increase their affordable housing portfolio within Salt Lake City.

4. Tracking the amount of interest In the OISL City program by the number of requests for applications is no longer an accurate measure of interest in the OISL program. In January 2003,CDC initiated their website
