This meeting is a discussion among Commissioners and select presenters. The public is
welcome. Items scheduled may be moved or discussed during a different portion of the
meeting based on circumstances or availability of speakers.
This meeting will be hybrid. The Commission Meeting will have a physical location at the
City and County Building in the Cannon Room 335 and members of the public may
observe the meeting and/or provide public comment in-person at this location.
Commissioners and presenters may attend and participate either in-person or through
electronic means.
Public Meeting Rules: The Council's meetings are a place for people to feel safe and
comfortable in participating in their government. A respectful and safe environment allows
a meeting to be conducted in an orderly, efficient, effective, and dignified fashion, free
from distraction, intimidation, and threats to safety. To support a respectful meeting,
comments or items that disrupt the meeting, intimidate other participants or that may
cause safety concerns are not allowed.

During this portion of the meeting, Commissioners may raise topics and discuss current
events that may impact or influence the Commission's overall work, discussions, and
deliberations to provide recommendations to the Mayor and City Council regarding
accessibility and disability matters in Salt lake City. No action will be taken during this
section of the meeting. Matters discussed during this section of the meeting may be
scheduled on a subsequent agenda for follow-up, if desired.
3.Public Comment (limited to 15 minutes):
Attendees may be provided one or two minutes of time, determined by the number of
attendees and the time available as determined by the Commission. Please observe the
time limit stated at the beginning of the public comment period so everyone may have a
chance to speak.
Per the public meeting guidelines, keep comments free of discriminatory language
referring to a person or group based on their religion, ethnicity, nationality, race, color,
descent, gender, sexual orientation, disability, age, or other gender identity factor. Items or
comments that disrupt the meeting, intimidate other participants or that may cause safety
concerns are not allowed.

9.TENTATIVE Closed Session
The Commission will consider a motion to enter into a Closed Session. A closed meeting
described under Section 52-4-205 may be held for specific purposes including, but not
limited to:
a. discussion of the character, professional competence, or physical or mental
health of an individual;
b. strategy sessions to discuss pending or reasonably imminent litigation;
c. discussion regarding deployment of security personnel, devices, or systems;
and
d. investigative proceedings regarding allegations of criminal misconduct.
A closed meeting may also be held for lawful purposes that satisfy the pertinent
requirements of the Utah Open and Public Meetings Act.

Residential Yard Collection Service Request
Need Assistance is only for garbage and recycling.
Please complete the enclosed form and return it to Salt Lake City's Waste and Recycling Division.The
Waste and Recycling Division is pleased to offer weekly, residential yard collection of garbage and
recycling to residents who are unable to bring their containers to the curb. In order to provide
residential yard collection efficiently, we must make sure that this service is provided only to qualified
residents. Qualified residents are those individuals who, because of a permanent or temporary physical
disability, are unable to bring the garbage and recycling carts to the curb, and who do not share a
residence with an able-bodied person who could move the waste receptacles. Containers must be
visible and accessible from a public right of way in order to provide the residential yard waste collection
service.
if you require residential yard collection because of a permanent or temporary physical condition, you
must have a medical professional or provide documentation, (see other forms of documentation) to
confirm your inability to bring your garbage and recycling containers to the curb.
The application will not be considered complete without a medical professional's signature or one of
the forms of documentation listed below, included with this application.
Other forms of documentation could include statements, records, or letters from a federal agency
issuing or providing disability benefits or a state vocational rehabilitation agency.
You or your medical provider may return the completed form by mail, email or fax which is listed at the
top of the form.

It shall be unlawful for any person to willfully misrepresent information on this form. A violation of this
section shall be a criminal misdemeanor subject to a penalty and/or imprisonment for each and every
offense.The Waste and Recycling Division at all times has the authority to terminate such service upon a
reasonable basis stated in writing to the recipient of the service. Upon termination of the service,the
individual must immediately use the roll-out container, brought to the curb, for service.
This verification is valid until such time as re-verification may be required by Salt Lake City's Waste and
Recycling Division Director.

The Accessibility & Disability Commission advises Mayor Erin
Mendenhall and the City Council on ways the City can
improve accessibility and the experience of the disabled
community.
We have members from:
• Disabled Rights Action Committee
• National Federation of the Blind of Utah
• Division of Services of the Deaf and Hard of Hearing
• Disability Law Center
• Utah Department of Workforce Rehabilitation
You can attend the meetings at the City & County Building on
the fourth Thursday of every month from 3:00 to 4:30 p.m.
