#1.Family Promise Salt Lake-Prevention and RRH Services

Description: In the spring of 2023,FPSL initiated a permanent Prevention and Stabilization Program
(RRH)aimed at keeping at risk families from becoming homeless.Utilizing funding from FPSL online
fundraisers,a grant from the Help Us Move In foundation(HUMI),and generous private donations,FPSL
was able to help 37 families facing eviction to remain housed.
Funding for this program would go toward direct client costs and administration

Description: As family homelessness continues to rise,FPSL has identified the need to expand
emergency shelter services by i00%in the spring of 2024.
Critical to meeting this goal is the acquisition of additional shelter sites and increased case management
aimed at securing affordable and sustainable housing for all program families.FPSL has located
additional shelter sites and are currently negotiating 2024 start dates.
Funds will go toward case management salary/fringe benefits,utilities,mortgage,and administration

#3.First Step House-Resource Center Program

Description: RCP embeds a behavioral health team in the Gail Miller Resource Center.The goal is to
improve access to behavioral health treatment for a population experiencing homelessness and untreated
substance use or mental health disorders.RCP staff work with homeless guests to get them into
appropriate treatment programs.They customize a pathway to treatment for each client,addressing
specific barriers such as benefits eligibility,records transfer,and even resistance to treatment. RCP is the
first program of its kind in Utah.No other homeless service program in Utah integrates SUD treatment
referrals as part of emergency shelter services.This collaboration helps FSH and The Road Home better
meet the needs of a population for whom untreated behavioral health conditions are a barrier to housing
and stability.The goal is to disrupt the cycle of recurring homelessness by addressing underlying stressors
for a population that otherwise slips through cracks in the homeless service system.
Funding will go toward salaries/benefits,direct program expenses,and administration

Description: UCA will staff case workers who receive clients from the current intake system and/or
partner referrals.Each client will be assessed and provided with holistic case management and
appropriate resources for their needs,which may include emergency rental or deposit assistance.Data
will be tracked on each client and reported to appropriate funders.

Description: It is critical that the Homeless Resource Centers(HRCs)are well and continuously
maintained,where services are provided without interruption so that a dignified and healthy environment
can persist in and around the centers for clients seeking services.The Homeless Resource Centers are a
community asset,as owner of the four HRCs,the goal is to keep the facilities well-maintained,clean,and
smoothly operating to best serve the vulnerable population that seek services in the facilities.
Scope of work includes regular repair and maintenance of the facilities Geraldine E King(GEK)and Gail
Miller Resource Center(GMRC).As well as supporting the cost of utilities,insurance,and the purchase of
an estimated go bed replacements.

#6.Soap2Hope-Harm Reduction Street Outreach Program

Description:The Soap2Hope program is designed to create self-sufficiency among survivors of
trafficking/sexual exploitation and reduce the recidivism rate of returning to that life.The program
provides street outreach,a diversion program for victims,reentry/post release services and referral to
community resources.The program works to end trafficking,sexual exploitation,and violence of women
in the Salt Lake City area.Services include referrals to drop-in centers and other trauma-informed
organizations,human trafficking prevention,outreach programs,detox and program options,and harm
reduction strategies.
Funding would go toward salaries/benefits,supplies,and transportation services

#7.The Road Home-Rapid Re-housing-FY2025

Description:TRH will help rapidly re-house eight literally homeless households by moving them into
apartments in Salt Lake County.Eligible families will receive 6 to g months of rental assistance depending
on their individual needs.These families will be referred to the program through partner agencies,or
shelters and outreach programs operated by TRH.TRH provides families with housing case management
services while in housing to provide them with other needed resources to help them achieve housing
stability.

#8.Volunteers of America,Utah-Youth Resource Center

Description:Youth experiencing homelessness are often trapped in a cycle of poverty and abuse;this
cycle can come at an extreme cost to the lives of youth and the broader community and has the potential
to be passed on to the next generation.The State of Utah Homelessness Data Dashboard reports that in
FY 22-23,there were 1,482 individuals between 18-24 in UHMIS for the Salt Lake CoC. Of those
individuals,98o were newly homeless and served through various outreach programs in Salt Lake
County.The percentage of unaccompanied youth who experienced previous instances of homelessness
was 54.9%•These alarming statistics highlight the critical issue of youth homelessness in Utah and the
need for accessible services that assist youth in healing from trauma and securing housing.
Funding will support utilities,laundry/housekeeping,building/grounds maintenance,and program
supplies

#9.Volunteers of America,Utah-Geraldine E. King Women's Resource Center
ESG
Description:To address the needs of women experiencing homelessness,the Geraldine E.King
Women's Resource Center provides 20o emergency shelter beds and essential services to 1200 single
women(aged 18 years and older)each year.Services provided through the program are rooted in the
Housing First Model and Trauma-Informed Care.The program addresses basic survival needs for shelter,
food,clothing,and hygiene. Daily program operations are accomplished by a professional team of about
50 full and part-time employees,and the facility is staffed 24 hours/day,365 days/year.The women
accessing shelter through the program are also provided Housing case management and access to housing
resources,connection to medical and behavioral health services through our community partners, and
other services to help them secure affordable and permanent housing.
Funding will support janitorial services,laundry/housekeeping,and program supplies
